Request approval to renew an existing contract - Senior Services, 23RFP138337A-CJC, Mobile Wellness Clinic Services at no cost with Grady Health System (Atlanta, GA) to provide Medicare wellness services, screenings, and education to Fulton County seniors (65+) with Medicare or public insurance. This action exercises the first of three renewal options. Two renewal options remain. Effective dates: January 1, 2025 through December 31, 2025. (APPROVED UPON ADOPTION OF THE CONSENT AGENDA)

Requirement for Board Action
In accordance with Purchasing Code Section 102-394(6), the Purchasing Department shall present all renewals requests to the Board of Commissioners at least 90 days prior to the contract renewal date or 60 days if the contract term is six (6) months or less.

Summary & Background: This partnership was created to assist Fulton County Seniors to be proactive in their physical and mental health needs, these mobile services at seven senior facilities and neighborhood centers will provide easy access to medical professionals and will help seniors by offering various preventive health services, enhancing their overall quality of life.
Scope of Work: This partnership will provide annual wellness assessment services to Fulton County Seniors at four senior multipurpose facilities and 3 neighborhood centers through mobile clinics to make preventive care more accessible to the senior population. This contract is structured to provide these services at no cost to Fulton County seniors who have Medicare or some form of public insurance, services include but are not limited to: Advanced Care Planning, Depression Screenings, Fall Risk Assessments, Neuro-Cognitive Assessments, Eye Screening Test and Health Education.
Community Impact: No negative community impact. The Mobile Wellness Clinic services can significantly improve health outcomes for seniors by making preventive care accessible within their communities, thereby enhancing their quality of life, reducing health disparities, and promoting overall community well-being.
