Request approval of a Resolution to accept the donation of blood pressure cuffs from the American Heart Association and approval of a contract between the County and the American Heart Association to facilitate receipt of the cuffs for provision to patrons of the Futon County Library System to eliminate health disparities and improve cardiovascular health efforts, access to healthy blood pressure resources, and high-quality patron education. This agreement is effective upon signature and will remain in effect for three years. This agreement has no fiscal impact. (APPROVED UPON ADOPTION OF THE CONSENT AGENDA)

Summary & Background
The purpose of this relationship is to create meaningful and sustainable improvements to the management of patron blood pressure, and cardiovascular health progress toward the goals and targets set in the AHA campaigns. The sharing of information about programs, products and services will improve health in the Fulton Community by concentrating on the three areas listed below:
1. Increase education and outreach in the community on heart disease, hypertension, and stroke.
2. Increase patient understanding and practice of self-measured blood pressure.
3. Spread key campaign messages regarding hypertension awareness and prevention, healthy lifestyles, and recognition of cardiovascular warning signs.
Scope of Work:
The AHA will carry out activities such as the following to achieve these goals:
• The AHA will assign a lead staff to function as the main point of contact between the AHA and the Fulton County Library System.
• The AHA will provide the Fulton County Library System with digital blood pressure cuffs and standardized education materials for patients most impacted by hypertension and who experience the greatest barriers to healthcare resources. Fulton County Library System will coordinate a blood pressure cuff access point in the library and/or loaner program. The cuffs are being provided by AHA in an “as is” condition with all faults, and AHA makes no warranties or guarantees of any kind related to the cuffs. Fulton County Library System acknowledges and agrees to the terms and conditions of the release attached hereto as Attachment A.
• The AHA will provide Target: BP posters in English and Spanish on accurate blood pressure measurement.
• The AHA will provide training resources to orient Fulton County Library System staff to the initiative, blood pressure education, and resources.
• The AHA will provide consultation and support to the Fulton County Library System in the development and implementation of AHA events, outreach activities, and messaging.
The Fulton County Library System will carry out activities such as the following to achieve these goals:
• Fulton County Library System, in collaboration with AHA, will establish a blood pressure screening hub and/or implement a blood pressure kit loaner program that provides patrons with automated blood pressure cuffs to be used in their homes for 14-28 days, and a patron information packet with resources on how to self-monitor blood pressure and how to achieve a healthy blood pressure level.
• Coordinate with AHA to identify 10 libraries to implement the blood pressure loaner program.
• Fulton County Library System will identify at least one person to serve as the AHA primary contact.
• Fulton County Library System will distribute blood pressure cuffs and educational materials provided by AHA to share with patrons. Fulton County Library System will distribute Target: BP posters to the public libraries.
• Fulton County Library System commits to providing timely and accurate reporting of the program activity to the American Heart Association monthly via conversations with the AHA point of contact. This information will consist of a number of kits borrowed from the public libraries.
• Fulton County Library System will share related AHA materials with patrons at appropriate library events.
